  2. Subject – Briefly describe your issue
  3. Service Impacted
    • Category - choose the area of issue
    • Additional Information – chose from the options available
  4. Severity – To help us understand the impact to you
    • General Guidance
    • Service Impaired
    • Feature Request
    • Server OutageContact number must be provided
  5. Server URL/Support Port No – Provide your portal domain 
  6. Description – Must all relevant information for your issue
  7. Attach a File
  8. Submit
